EVIDENCE OF SALE OR DELIVERY OF DISEASED POTATOES
We, __________________________________________________________________________
(name and address of producer, dealer or carrier)
inform the purchaser or consignee that the potatoes sold or delivered
have been the subject of a notice by an inspector of the Ministère de
l’Agriculture, des Pêcheries et de l’Alimentation to the effect that
they are infected with the following disease:
( ) bacterial ring rot;
( ) spindle tuber;
( ) other: ___________________________________________________________________
(specify the disease)
Therefore, you must not use, sell or deliver any of these potatoes for
planting.

another shall be given to the purchaser or consignee and a copy shall be
